R27 Cold Start Nightmare

My .02, and my experience with my R26, is that these engines really vibrate, to the point that spark plugs have rotated loose, and that the exhaust collar rotated loose as well (I tend to under-tighten these things, probably not the best practice).

I'd check all electrical connections, including plug caps on the plug wire, etc.

I'd also check to see the valve train adjustment, because those could also vibrate out of position.
