Heel toe shifter for 1968 R60/2

I have R60/2 with a Dnepr sidecar attached. Due to a medical issue, drop foot, I can no longer shift using the stock shifter. Does anyone have experience with fabricating a shifter with a heel toe configuration? Due to the throw of the kickstart it appears that the shifter would need to be curved in such a way as go below and behind the kickstart. An unusual feature is that the shaft leaving the transmission to which the shifter attaches on my bike has a splined end. Any help is appreciated.

So you can push down with your left toe and left heel, but you can't pull up with your left toe, right? I'm not there yet but the neurosurgeon wants to fuse my lumbar vertebrae anyway. Getting old ain't for sissies , eh?

Depending on how talented your fabricator is, if you hinged the heel pedal so it could swing up out of the way for starting and then back down for riding you'd be all set.

Correct left foot description. Thank you for your time and suggestion. A possible solution I had not considered.
